Live Gaussian Blur: You will see noise on the edges, depending on the brightness of your subject. I believe it should be a default setting, while the Lens Blur tool might be a useful intermediate step. Focus Enhance, while a bit unsophisticated for most, may be useful for those still uncomfortable with Photoshop’s Refine Edge tool.
